Aspirin & Macular Degeneration (AMD)

There may be a relationship between frequent aspirin use and Macular Degeneration (AMD) according to researchers from the European Eye Study who reported some interesting findings on the October 2011 publication Ophthalmology which is the official journal of the American Academy of Ophthalmology. They found that frequent aspirin use may be associated with an increased risk of early or late “wet age-related macular degeneration”, with an “odds ratio” that increases upon frequency of consumption. However the study is somewhat limited in that there was an unknown amount of aspirin taken, as well as the possibility that participants may have taken aspirin after experiencing visual problems. So, at this time the study is interesting but inconclusive and certainly patients taking aspirin to offset the coronary risk profile or other vascular problems should NOT discontinue taking aspirin unless they have been directed to do so by their personal physician.

Why NOT to Buy Eyewear or Glasses Online

Did you know that in Massachusetts the state regulates that eyewear is a medical device and by law it is required to be fit and dispensed (provided) by a qualified licensed Optician or a registered apprentice? On-line businesses do not follow this regulation, however they are permitted to provide eyewear because they are based outside of state and/or country lines. At D’Ambrosio Eye Care there are several measurements and considerations when customizing a pair of eyewear that includes a monocular pupillary or visual center measurement (VCP), your visual needs, prescription, lens type, frame style, wrap angle, vertex distance, pantoscopic tilt, base curve, head and facial features, the intended use for your eyewear and previous eyewear worn. When fitting eyewear we also make precision adjustments so you are looking through the optical center of the lens. Our Opticians also want your frame to fit properly so they do not slip on your nose, pinch the sides of your head, leave red marks or cause discomfort behind your ears or on your nose. Any of these concerns may cause headaches and/or decreased vision. The human face is not symmetrical left to right so using only your binocular PD will not place the pupils in the visual center of the lens and possibly cause distortion, unwanted prism and headaches. Taking this measurement yourself- as some online stores request you do is tricky-somewhat akin to trying to cut your own hair.

Finding the Best Cataract Surgeons in Massachusetts

To find the best cataract surgeons requires a little bit of work and investigation but is always worth it. Finding an eye surgeon who is a cataract specialist can help you to be confident that you are getting the most current information, thoughts and techniques to deal with your cataracts.

Ask People You Trust for a recommendation. Ask your friends, co-workers and family-but most importantly ask you primary care physician who they would go to or who they would send a parent to for cataract surgery.

Don’t Limit Yourself to Insurance Lists. Toric Contact Lenses for Astigmatism

Toric Contact Lenses for Astigmatism can be very helpful for patients. Many people suffer from blurred vision caused by astigmatism. Astigmatism occurs when the cornea or the natural lens of the eye are unable to focus a clear image due to an irregular surface. Most people with astigmatism have an oval cornea, shaped like a football, instead of round like a basketball. In the past, patients may have been told that they could not wear contact lenses because they have astigmatism. This is no longer the case. Almost all cases of astigmatism can be treated with some type of contact lens.

Soft toric contact lenses are the most popular type of contact lens used to treat astigmatism. Good initial comfort is the main advantage of this type of lens, and the availability of trial contact lenses helps the initial fitting process. Soft Toric lenses are available in a disposable modality so long term convenience, health, and comfort are improved.

Rigid gas permeable (RGP) contact lenses are also used to treat astigmatism. Patients will have more initial awareness when wearing rigid gas permeable lenses compared to soft lenses, but long term comfort is usually good after the patient becomes accustomed to them. RGP contact lenses are able to treat greater amounts of astigmatism than soft lenses.

Hybrid contact lenses have a rigid gas permeable center and a soft lens skirt surrounding it. This type of lens tries to combine the sharp vision potential of a rigid lens with the comfort of a soft lens. About Your Eyes & Being Pregnant

What does being pregnant have to with your eyes? Sometimes quite a bit. During pregnancy your body undergoes fluctuations in hormone levels, changes in fluid retention and even an increase in your overall blood volume. These types of changes can result in your eyes changing in various ways. Fortunately eye changes due to pregnancy are usually temporary and resolve after the baby is born or after the cessation of breast feeding. Typically, vision changes are minor and don’t require a change in eyeglasses, however there are some eye changes that do require care and attention.

If you were thinking about having LASIK you should delay your actual LASIK Surgery procedure until at least three months after your delivery or three months after you stop nursing. This is necessary because the thickness of your cornea may fluctuate during pregnancy and will reduce the accuracy of the LASIK correction. Also, hormonal fluctuations are often the cause of dry eye. This can make you uncomfortable by itself and can certainly make wearing your contact lenses more difficult. If you experience dry eyes during pregnancy be sure to consult your eye care provider so that “pregnancy-safe” lubricating eye drops or other alternative treatments for dry eye can be prescribed. Sometimes simply eating foods high in omega-3 fatty acids such as salmon, flax seeds and walnuts may help resolve dry eye and also supports general good health.

Eye Turns or Strabismus

Eye turns (strabismus) in children can have multiple repercussions. “There are a number of functional problems that kids can have with strabismus (eye turns). If one eye turns regularly, the vision may not develop properly in that eye; if a turn is always present, depth perception will not develop. Another important, but often overlooked, negative effect from strabismus has to do with the social impacts that an eye turn has on children” stated Oren L. Weisberg, M.D., Pediatric Ophthalmologist at D’Ambrosio Eye Care.

Dr. Weisberg continued, “A recent article reinforces our understanding of this. In the study, pictures of children were digitally altered to give them an exotropia (an eye turn outward). A group of children aged 8-12 years then looked at the pictures and were asked to rate their willingness to sit next to such kids compared to pictures of kids without eye turns. Kids of all age groups studied were less willing to sit next to a child with exotropia. Other studies have also shown that strabismus can negatively affect kids in social situations”.

There are treatments available for patients with strabismus. This can include eyeglasses, patching, eye drops and sometimes surgery. LASIK for Contact Lens Problems & Intolerance

“Contact lens problems and intolerance seem to be motivating more and more patients to schedule LASIK consultations these days”, said Massachusetts Corneal Specialist JoAnn C. Chang, M.D. of D’Ambrosio Eye Care.

In fact, contact lens problems and intolerance are the reasons a great many patients seek LASIK Surgery and Laser Vision Correction. “The most common reason for contact lens intolerance that we see is a condition called Giant Papillary Conjunctivitis or GPC”, commented Dr. Chang.

When we wear contact lenses, no matter how successful or diligent we are in their care and replacement, they become coated with mucous and protein from our tears. After a number of years of wearing contacts it is not uncommon to develop an allergy to the protein on the contact lens. Initially this may result in patients having some dry eye symptoms and prompt them to use lubricating eye drops. However, as the GPC contact lens problem continues to progress, patients begin to notice some itch and stringy mucous type discharge from their eyes. After a while the contacts just become too uncomfortable and gritty and patients become intolerant and just can’t wear their contacts.

During a severe episode of GPC, patients may be restricted from wearing their contacts in order to reduce the allergic inflammation of the lids. In some cases, patients are no longer able to wear contacts again at all. Patients with a chronic GPC may decide to have LASIK to correct their vision, so that they no longer need to depend on contacts on a daily basis.

Detached Retina-What Causes It?

You may have heard of someone having a detached Retina and wondered what that is and what might have caused it to occur.

First, let’s talk about the Retina. The Retina is located at the back of the eye and is made of many layers of cells and nerves that sense light and capture a picture. The picture then is sent to the brain. The eye is like a camera, with the light being focused by the front of the eye. Like a camera that uses film, the retina is like the film lining the back of the camera with the lens focusing light onto it.

When the Retina detaches, it loses its normal position and separates from the wall of the eye. When it is not in the proper position it cannot function properly and vision will be blurred. This is a serious problem, a blinding eye problem, and needs to be treated to try to prevent permanent blindness.

So, how does the Retina become detached? There are a few different causes.

The middle of your eye is filled with a clear gel called the vitreous and it sits close to the retina. Over the years, the vitreous liquefies, in both young people and old. It often changes position and separates away from the Retina. This generally doesn’t cause a problem. Sometimes it may pull away and cause a hole or a tear the Retina. This tear can occur in one or more places. This can allow fluid to pass through the hole or tear, causing the retina to lift away from the back of the eye, similar to wallpaper peeling from a wall.

Are there any warning signs? New flashing lights, new floaters, or a shadow in the side of your vision could be the first signs of a retinal tear or retinal detachment, but not always. If you see any of these, it is recommended you contact your ophthalmologist for further examination.

Glaucoma Awareness Month in Massachusetts

D’Ambrosio Eye Care wants to focus patient’s attention on glaucoma this month as January is National Glaucoma Awareness Month. “This is an important time to spread the word about this sight-threatening disease. Our understanding of this disease, along with the ways in which we can diagnose and treat it, has improved considerably,” commented Massachusetts Glaucoma Specialist Bin (Benjamin) Wu, M.D. of D’Ambrosio Eye Care.

Glaucoma is the leading cause of preventable blindness. Moreover, among African American and Latino populations, glaucoma is the leading cause of blindness overall. Of particular note is that glaucoma is 6 to 8 times more common in African Americans than Caucasians.

Over 4 million Americans, and nearly 70 million people worldwide, have glaucoma. Experts estimate that half of them don’t know they have it. Combined with our aging population, we can see an epidemic of blindness looming if we don’t raise awareness about the importance of regular eye examinations to preserve vision.

The most common type of glaucoma—Primary Open Angle Glaucoma—is hereditary. The Nottingham Glaucoma Study published in the British Journal of Ophthalmology evaluated the risk that siblings of glaucoma patients would themselves develop glaucoma within their lifetime. “While we already knew that there was a strong likelihood that family members of glaucoma patients were at greater risk, the Nottingham Study found that siblings were 5 times more likely to develop glaucoma by age 70. This is why we strongly recommend that siblings of glaucoma patients and glaucoma suspects be screened for glaucoma, each and every year,” said Dr. Wu.

Multifocal Lens Implants (IOL) at D’Ambrosio Eye Care

Intraocular lenses, or IOLs as they are more commonly known, are artificial lenses that are used to replace natural lenses that have become clouded with cataracts. They can also be used as a solution for people who suffer from presbyopia, a condition in which the lens becomes less flexible, thus losing its ability to focus on near objects. Before the U.S. Food and Drug Administration approved the use of intraocular lenses in 1981, patients who had cataract surgery were forced to wear thick eyeglasses or contact lenses to improve their vision.

Lucentis for Diabetic Eye Disease

Reports of a two-year clinical study of the anti-vascular endothelial growth factor (VEGF) drug Lucentis when used to treat Diabetic Macular Edema (DME) showed that it helped eliminate the problem which is a cause of significant vision loss in patients with diabetes. Patients with Diabetic Retinopathy in the form of DME who received the drug experienced rapid and sustained improvement in vision compared to those who received a placebo injection.


Diabetic Macular Edema or DME is the swelling of retina in people with diabetic eye disease called diabetic retinopathy, which causes damage to the blood vessels of retina. The DME patients suffers fluid leak from the damaged blood vessel to the central portion of retina, causing it to swell leading to blurred vision, severe vision loss and blindness.

Of the 26 million U.S. people with diabetes, a portion of up to 10 percent tends to develop DME during their lifetime, and up to 75,000 new cases of DME are estimated to develop each year. Currently, DME patients are treated with laser surgery that helps seal the leaky blood vessels to slow the leakage of fluid and reduce the amount of fluid in the retina.

Lucentis is approved for use by the U.S. Food & Drug Administration (FDA) for the treatment of “Wet” Age-Related Macular Degeneration (AMD) and for Macular Edema following Retinal Vein Occlusion. In 2011, Lucentis was approved for treatment of visual impairment due to DME in Europe.

Recovery After Cataract Surgery

Cataract surgery today is really very advanced and quite “patient friendly.” At D’Ambrosio Eye Care we perform our Cataract Surgery and Lens Implants at the Cataract and Laser center of Gardner. This eye surgery facility is very comfortable and efficient and your entire Cataract Surgery procedure should take only 2-3 hours from the time you check in until the time you check out.

Second, advanced cataract surgery is most often performed with local or topical anesthesia. Thus, there is really minimal if any “recovery” from the anesthesia used. You are usually feeling “back to normal” by the end of the day of your surgery or certainly by the next morning.

Third, with the advanced Cataract Surgery technique of “small incision phacoemulsification” there is only a need to create a tiny incision-often just a few millimeters-through which the Cataract is removed and the intraocular lens is implanted. Thus the wound healing is very quick.

Finally, depending on the type of lens implant that is used it is highly likely that your distance vision will be dramatically improved within 24 hours. In fact many patients actually are comfortable driving the next day after their cataract operation. If you have a multifocal or accommodating lens implant to correct both your distance and near vision, it might take a bit longer to appreciate the full clarity of the near vision correction. So, it is pretty likely that you will be back to work in a few days. Depending on your cataract surgeon’s instructions, you should be able to resume all of your normal activities with a couple of days.

Diabetic Eye Disease Month at D’Ambrosio Eye Care

D’Ambrosio Eye Care wishes to announce that Prevent Blindness America has designated November as National Diabetic Eye Disease Awareness Month. This is an important time to spread the word about this potentially blinding disease. We need to work to help patients avoid the complications of diabetic retinopathy, which blinds over 8,000 Americans each year. The vision loss from diabetic retinopathy can be prevented if it's caught and treated in time. A recent study found that more than one third of those diagnosed with diabetes do not adhere to vision care guidelines recommending a dilated eye exam every year.

As part of Diabetic Eye Disease Awareness Month we are urging people with diabetes to have a dilated eye exam every year. The longer a person has diabetes, the greater his or her risk for developing diabetic retinopathy. However, diabetic retinopathy does not only affect people who have had diabetes for many years, it can also appear within the first year or two after the onset of the disease.

Patients can help to reduce the risk of developing diabetic eye disease by not smoking, controlling their cholesterol and lipid profile and blood pressure, as well as working to eat a heart-healthy diet rich in fish, fruit and green leafy vegetables and exercising.

Eye Safety for Halloween

Halloween has become a favorite time of year for folks young and old to ‘dress up’ and along with that comes the need to remind adults and parents of some eye safety tips.

1. DECORATIVE CONTACT LENSES – sometimes referred to as theatrical or costume lenses, it is important to remember these are medical devices and unless prescribed by an eye care professional, avoid them. They are regulated by the FDA, but many people buy them in salons or flea markets and they can do more harm than good to your eyes. Whether you need corrective lenses for vision or not, these must be prescribed by an eye care professional they can damage your eyes without a proper fitting.

2. MASKS – masks may be an important part of your costume, but use caution to ensure that your visibility is not impaired. Pirate costumes are popular for all age groups, but using only one eye to see can be dangerous.

If you or your child must wear a mask, make certain the eye holes are large enough to ensure good visibility. This may mean making the holes larger, but it will be worth the effort to help with safety.

Parents should think of an alternative using makeup to aid their children’s ability to see properly while still enjoying their outing.

3. WIGS & HATS – can impair your vision so use caution when using either of these. They can slip or turn and block your field of vision so make certain they are securely in place.

4. MAKEUP – while makeup is a good alternative to a mask, patients should check to ensure that it is hypoallergenic and FDA approved. Also be careful to avoid the eye area as it could cause an irritation to this sensitive area.

Use caution as well when applying false eyelashes as the glue can get into the eye area and cause damage and irritation.

5. PROPS – if part of your child’s costume is a sword, wand or anything stick-like, make sure it is made of a flexible material so they don’t injure themselves or others.

6. COSTUMES – while an all black (or dark) costume may be perfect for your children’s character, it makes them very hard to see at night or in a crowd. At a minimum, make certain to have reflective tape on their clothes.

7. TREATS – it’s important that children understand they should not eat anything on their route but should bring it home for an adult to inspect. Any items that are not age-appropriate (peanuts, small candies) for the child or if it appears tampered in any way, should be thrown away. Better to err on the side of caution than to have a problem.

Eye Injury Prevention Month at D’Ambrosio Eye Care

D’Ambrosio Eye Care wishes to announce that October has been designated Eye Injury Prevention Month by the American Academy of Ophthalmology. When most people go out to mow the lawn, jump-start the car or get ready to do some spring cleaning, the last thing they think about is protecting their eyes, but it should be first on their mind. It is a good idea to think about protecting your eyes from the extraordinary damage often caused by the most ordinary of activities.


Many household chemicals, such as cleaning fluids, detergents and ammonia, are extremely hazardous and can burn the eye's delicate tissues. When using chemicals, always read instructions and labels carefully, work in a well-ventilated area and make sure spray nozzles point away from you and others before spraying. Be sure to wash your hands thoroughly after use.


Before using a lawnmower, power trimmer or edger, check for debris. Stones, twigs and other items can become dangerous projectiles shooting from the blades of a lawnmower, potentially injuring your eyes or those of innocent bystanders. Prevention is the first and most important step in avoiding serious eye injuries, so be sure to protect your eyes with appropriate protective eyewear.

Continuing Nursing Education Offered Through D'Ambrosio Eye Care

Recently, D’Ambrosio Eye Care has provided contact hours for nursing credit for Registered Nurses and Nurse Practitioners in Massachusetts. Francis A. D’Ambrosio, Jr., M.D. and JoAnn C. Chang, M.D. presented information to two large groups of nurses on “Common Eye Problems for the Primary Care Practitioner.” Dr. D’Ambrosio, Jr. recently stated “many nurses find they did not have much exposure to diagnosis and treatment of common eye problems and this course was developed to assist them in their day-to-day activities”.

Dr. D’Ambrosio, Jr. presented on triage of eye problems covering topics from eye injuries, inflammation and pain to floaters. Many of the attendees expressed their lack of specific knowledge in areas impacting the eye and were very happy to receive this information. The triage presentation afforded nurses the opportunity to learn whether a specific incident would require immediate treatment or urgent treatment and follow up by an ophthalmologist. Within a few days of the course one of the nurse practitioners was presented with a patient that had one of the less common problems and was accurately able to refer the patient for immediate treatment and follow up care. Without the course, she might not have recognized the urgency of the problem.

In addition to this course, Dr. D’Ambrosio Jr. has hosted several nurses in our clinic for additional continuing education hours. The programs are specifically tailored to each nurse and their area of interest. Corneal Specialist JoAnn Chang, M.D. of D’Ambrosio Eye Care

D’Ambrosio Eye Care Ophthalmologist, LASIK & Cornea Specialist, JoAnn C. Chang, M.D., diagnoses and treats patients with cataracts and diseases of the cornea.

Dr. Chang graduated from The University of Cincinnati College of Medicine (with Honors) in Ohio and continued with her residency at The University of Chicago Hospitals. Your risk of developing Cataracts may be higher if you are being treated for prostate cancer with Androgen Deprivation Therapy (ADT). The side effects of ADT such as weight gain, insulin resistance and blood lipid level problems have been linked to Cataract formation. Children's Eye Health Month at D’Ambrosio Eye Care

D’Ambrosio Eye Care wishes to announce that August has been designated as Children’s Eye Health and Safety Month by Prevent Blindness America. Eye and vision problems affect one in twenty preschoolers and one in four school aged children. Parents should be aware that it is possible for their children to have a serious vision problem without even being aware of it. Infants should be screened for common eye problems during their regular pediatric appointments and vision testing should be conducted for all children starting at around three years of age. If there is a family history of eye problems or if an eye problem is apparent, it is important to bring it to the attention of and eye doctor so that they can advise the parents about when and how often their child’s eyes should be examined.

Among the conditions an eye doctor will look for are amblyopia (lazy eye), strabismus (crossed eyes), ptosis (drooping of the upper eyelid), color deficiency (color blindness) and refractive errors (nearsightedness, farsightedness and astigmatism). Contact Lens Wearers and Non Compliance

Recent results from two on-line surveys sponsored by VISTAKON®, Division of Johnson & Johnson Vision Care, Inc. indicate contact lens wearers are significantly non-compliant in many areas of contact lens wear and care. One survey addressed the frequency of lens replacement and the other posed questions regarding disinfection of the lenses, hygiene and the storage case replacement.

JoAnn C. Chang, M.D., Corneal Specialist at D’Ambrosio Eye Care, Inc. was not surprised by the results of these surveys. Dr. Chang states “the cornea is the window of the eyes, it must remain clean and clear for optimal vision.” Recently, Dr. Chang saw a young woman in the Lancaster office of D’Ambrosio Eye Care from over-wearing contact lenses. While Dr. Chang was able to save her patient’s eye, unfortunately this young woman has lost a significant portion of her vision in one eye. The contact lens was left in place too long and managed to mask the abrasion on her cornea and became a bacterial breading ground. The infection spread into her eye. “It is critical for contact lens patients to remove their lenses as directed by their eye care professional and to maintain proper hygiene and replaces lenses when instructed to attain good eye health” according to Dr. Chang.

Eye Imaging Technology at D’Ambrosio Eye Care

Recently, the Lancaster location of D’Ambrosio Eye Care acquired a new, high resolution imaging camera to help us record eye problems and conditions digitally. This camera is able to take photos of the optic nerve and optic disc through a non-dilated, 3mm pupil with great results. With the combination of optics, digital sensor and computer combined into one, it makes it easier for patient comfort and for staff. This will give D’Ambrosio Eye Care doctors greater ability to capture high resolution, non-mydriatic color images and perform fluorescein angiography with just one piece of equipment.

Meet Glaucoma Specialist Bin Wu, M.D.

D’Ambrosio Eye Care is pleased to have Dr. Bin Wu, a specialist in diseases and surgery pertaining to glaucoma. Dr. Wu attended Medical School in China at the En Shi Medical College and Tongji Medical University. Upon completion of his studies there, he performed his residence at the Interfaith Medical Center, followed by a Fellowship at the Indiana University Medical Center in glaucoma. Dr. Wu specializes in diseases and surgery of the anterior segment (inside) of the eye including the medical, laser and surgical treatment of glaucoma. Dr. Wu is a member of the American Academy of Ophthalmology, the American Society of Cataract & Refractive Surgery and the Association of Research in Vision and Ophthalmology.

Near Vision & LASIK Monovision

LASIK-which is mainly a type of laser eye surgery for correcting distance vision, can be used for correcting near vision through a technique called monovision LASIK. Monovision corrects one eye for distance and one eye for near. Thus, the need for glasses with two different focal lengths is resolved by focusing each eye at a different focal length. Monovision LASIK can be effective for some patients with the need for mild near vision correction. Depending on the amount of close work you do at work and for how long each day, monovision LASIK may be a good alternative. Fireworks Eye Safety Awareness Month at D’Ambrosio Eye Care

D’Ambrosio Eye Care wishes to announce that Prevent Blindness America has designated July as National Fireworks Eye Safety Awareness Month. Contrary to what you might think, there is no safe way for nonprofessionals to use fireworks. It is only safe to enjoy the splendor and excitement of fireworks at a professional display. According to the U.S. Consumer Product Safety Commission, fireworks are involved in approximately 10,000 injuries per year that are treated in U.S. hospital emergency rooms. Of those, 2/3 occurred during the one month period surrounding the July 4th holiday. It is important to note that

• Eyes were the Second Most Commonly Injured Part of the Body!
• Males Were Injured 3x More than Females.
• Approximately 25% of Injuries are to Children under 15.

Fireworks and celebrations go together, especially during the Fourth of July. But there are precautions parents can take to prevent these injuries. The best defense against kids suffering severe eye injuries and burns is to not let kids play with any fireworks. Do not purchase, use or store fireworks of any type. Protect yourself, your family and your friends by avoiding fireworks. Attend only authorized public fireworks displays conducted by licensed operators, but be aware that even professional displays can be dangerous. If an eye accident does occur, what should I know?
• Do not rub the eye.
• Do not attempt to rinse out the eye. This can be even more damaging than rubbing.
• Do not apply pressure to the eye itself- but protecting the eye from further contact with any item, including the child's hand.
• Do not stop for medicine! Over-the-counter pain relievers will not do much to relieve pain. Aspirin (should never be given to children) and ibuprofen can thin the blood, increasing bleeding. Take the child to the emergency room at once.
• Do not apply ointments or drops. They may not be sterile and may make the area around the eye slippery and harder for the doctor to examine.

LASIK Safety at D’Ambrosio Eye Care

If you or someone you know is considering LASIK you will want to know that the procedure is safe and has a solid overall track record of safety so that they can overcome any fear or anxiety they might have regarding the procedure. The safety of LASIK depends on a number of factors. By far the most important factors that can affect LASIK safety are the laser and instrumentation to be used to perform the treatment and even more so, the skill and experience of the LASIK Surgeon.

Long Term Track Record
Laser Eye Surgery for the Laser Vision Correction of nearsightedness was first performed in the United States upon Food and Drug Administration (FDA) approval of the Excimer Laser on October 20, 1995. The Excimer Laser was specifically FDA approved for LASIK in 1998. So as of 2011, Laser Eye Surgery for Laser Vision Correction has been performed for 16 years and the LASIK procedure has been performed under FDA approval for more than 13 years. It is estimated that some 20 million patients worldwide have had LASIK. LASIK is generally considered one of the most successful and safe surgical procedures of any type. In skilled hands nearsighted patients can expect to achieve 20/40 vision more than 98 percent of the time and uncorrected vision of 20/20 or 20/25 in more than 90 percent of the time. The safety record is equally as impressive with regard to any loss of best-corrected vision where vision loss to a level of worse than 20/40 is quite rare occurring in about only 3 per 1,000 cases. Serious complications such as infection or corneal damage, occur even more infrequently in fewer than 1 in 1,000 cases. There is more to safety than simply the laser itself.

Safety is the Hands of the LASIK Surgeon
FDA approval provides some assurance but FDA approval does not indicate that a LASIK Surgeon is going to provide a thorough evaluation and consultation nor does it imply that LASIK Surgeons will provide a complete review of the possible risks and complications of LASIK or even that a LASIK Surgeon will use the appropriate screening and decision criteria to be sure that a patient is in fact a good candidate for LASIK or any Laser Eye Surgery for that matter.

With regard to LASIK safety, choosing a LASIK Surgeon is the most important decision a patient makes in deciding to have LASIK and is a significant part of making LASIK a safe and effective procedure. You should not choose a LASIK Surgeon based on slick advertising or low price. You should choose a LASIK Surgeon based on training and experience, reputation in the community and the comfort and rapport established during your consultation. It is ALWAYS the skill and experience of the surgeon that contributes the most to the overall safety of Laser Eye Surgery for the correction of nearsightedness, farsightedness and astigmatism. SPOTLIGHT on Sahana Vyas, M.D.

Sahana Vyas, M.D. is the Retina and Macular Degeneration Specialist at D’Ambrosio Eye Care. Dr. Vyas sees patients in their Lancaster office.

Dr. Vyas received her undergraduate degree from Harvard University and graduated from Saba University School of Medicine. She did her internship at Brown University – Rhode Island Hospital and her residence at the University of Louisville (her home town) – Kentucky Lions Eye Center. She completed her fellowhip in vitreoretinal diseases and surgery at the University of Virginia.

She has been with the practice nearly five years and she sees patients with diabetes, retinal vascular disease and macular degeneration. Dr. Vyas has a very gentle touch with her patients which they greatly appreciate! Her technicians are also very caring and helpful with patients.

Tired of Burning, Dry Eyes?

“We are happy to once again be participating in an FDA study to assess candidates for a future dry eye study”, said Francis A. D’Ambrosio, Jr., M.D., Medical Director of D’Ambrosio Eye Care with offices in Lancaster, Gardner and Acton, MA.

Dry eye is the most prevalent form of ocular discomfort and irritation with millions of people affected with mild to moderate dry eye. It can be related to external factors, such as the low humidity of air-conditioned offices, dry heat in the winter, windy and dusty outdoor conditions, computer use for prolonged periods of time or the wearing of contact lenses. There are also internal factors; such as hormonal imbalance, autoimmune disease, use of prescribed systemic medication, anatomical changes or trauma and aging.

These symptoms result in, at a minimum, decreased quality of life, and in severe cases, pain, light sensitivity and impaired vision. Dr. D’Ambrosio, Jr. stated “as the population in the United States ages, accompanied by increased computer use, the evidence of dry eye will only increase”.

Assessment appointments will be held at D’Ambrosio Eye Care located at 479 Old Union Turnpike in Lancaster, MA, beginning in June, 2011.

We are looking to assess individuals based on the following criteria:

* Must be age 18 or older
* Have a history of dry eye in both eyes
* Willingness to go to D’Ambrosio Eye Care in Lancaster for appointments, if selected for the study

The initial assessment appointment will be between 1 – 2 hours in Lancaster, MA and individuals will be compensated for their time. If accepted into the study, it will begin in the Fall of 2011.

Is LASIK or PRK Better?

“Both LASIK and PRK are excellent choices for the appropriate patients”, commented JoAnn C. Chang, M.D., Cornea Specialist and LASIK Surgeon at D’Ambrosio Eye Care. Here are some considerations for you to know and understand about PRK vs. LASIK. First, the long-term clinical studies comparing PRK and LASIK show that the ultimate visual results of both types of laser treatment are virtually identical. However there are some subtle differences as well as some not so subtle difference.

“Patients who have LASIK may have very slightly reduced contrast sensitivity in dim illumination”, said Dr. Chang. “May” is the operative word here. If one uses extremely sensitive clinical measuring methods they can sometimes find a slight reduction in “low contrast” vision. Often it is so slight that the patient does not notice it except in the testing conditions or in very extreme instances of reduced contrast situations such as driving at dusk in the rain. Thus it is a limited problem for most patients.

The real differences between PRK and LASIK are based on the fact that PRK is a “surface laser treatment” and LASIK is a “lamellar laser treatment”. PRK is performed on the surface of the cornea whereas LASIK is performed between the layers of the cornea. To do a LASIK it is a 2-step procedure-one to create the “layer” or “flap” and one to apply the laser. To do a PRK is a single step procedure, just applying the laser. Thus, whenever you have a multi step eye surgery procedure there can be a greater possibility of risks, complications or side effects. “Fortunately the risks, side effects and complications of LASIK are relatively few and minor if you are indeed a good candidate and under the care of a top laser surgeon”, said Dr. Chang. So, PRK does offer a safer risk profile because it is a single step procedure. However one of the not so subtle differences is that because it is a lamellar procedure, LASIK does not disturb the surface of the cornea during treatment. This provides patients with a much faster visual recovery and significantly less discomfort than PRK. This is why from a consumer perspective patients have tended to want LASIK eye surgery more than PRK. They can get back to pretty many normal activities within a couple of days without any real pain-just some mild discomfort-and with good vision. Most LASIK patients are able to pass a drivers test with 24-48 hours of their laser treatment. PRK patients often experience moderate discomfort that can last a few days and depending on the degree of their prescription can take a week or so to achieve good functional comfortable vision. About 20.5 million Americans age 40 and older have cataracts and more than half of all Americans develop cataracts by age 80.

Many patients do not actually know that a cataract is a clouding of the crystalline lens of the eye preventing light rays from passing through it easily. This results in a clouding and blurring of vision. They need to understand that cataracts are not a growth or a film over the eye. For many patients cataracts start out slowly and have little effect on vision at first. But, as the cataract becomes denser, so does the impact on vision. The most common symptoms that bother patients with cataracts include:

• blurring of vision
• sensitivity to light and glare
• double vision in one eye
• poor night vision
• fading or yellowing of colors
• frequent changes in glasses or contact lens prescriptions.

Memorial Day is the official kick-off to summer and brings families and friends together. It’s also a time when folks are doing activities they may not have done for several months and it requires a little planning. Childrens’ Eyeglass Painless Growth Program

D’Ambrosio Eye Care is proud to offer a special “Parent’s Painless Growth Program” in our Optical Departments to assist the parents of children who wear eyeglasses.

Program Details:
Throughout your child’s life their vision is growing and changing, much like the size of their clothes or shoes. For many children, their prescription can change more than once each year until their eyes are fully developed. In cases like this, D’Ambrosio Eye Care recognizes that purchasing a new pair of lenses with each change could become costly to parents, so we offer your child free lens changes for these premature changes that happen earlier than routine eye examinations (up to age 18).

Who Is A Good Lasik Candidate?

Dr. Chang has relayed the following guidelines for patients to consider regarding who is a good candidate for LASIK surgery:

• The great majority of patients who are nearsighted, farsighted or have astigmatism are potential candidates for LASIK.



• Good LASIK candidates are at least 18 years of age or older and have stable vision. This means that their eyeglass or contact lens prescription has not changed much in the last year or so. Many people do not have "stable" vision until 21 years of age, however so each person is a bit different.




• Good LASIK candidates have healthy eyes that are free from retinal problems, corneal scars, and any other eye diseases. They have normal corneal shapes and thickness as well as healthy tears.




• The best LASIK candidates are those with a lifestyle, activities or occupation in which they are dissatisfied with their contact lenses or glasses.

Ultraviolet (UV) Eye Protection Awareness Month

D’Ambrosio Eye Care wishes to announce that Prevent Blindness America has designated May as National Ultraviolet Eye Awareness Month. Most people know the harmful effects that ultraviolet (UV) rays can have on the skin. But many are not aware of the damage that they can cause to the eyes. Possibly the most frightening aspect of UV damage is that it is cumulative, meaning the negative effects may not present themselves until years later.

Today almost everyone is aware of the potential damage that sunlight and ultraviolet (UV) radiation can cause to their skin, most do not know or appreciate that ultraviolet light (UV) can also cause significant damage to eye health and vision.

“Most of us wouldn’t dream of staying outside in the sun without putting on sunscreen lotion,” said Francis D’Ambrosio, Jr., M.D., Medical Director of D’Ambrosio Eye Care. “But we also need to remember to wear both UV-blocking sunglasses and a brimmed hat to protect our eyes as well.”

Patients are fortunate to have wide range of UV protecting eyeglass lenses available to choose from. Eye protection from UV needs to be complete but doesn’t have to be overly expensive. High quality sunglasses should block out 99-100 percent of both UV-A and UV-B radiation. It is also important to have UV protection in everyday eyewear, which is readily available with a number of UV blocking eyeglass lens materials, specialized coatings and photochromic lenses.

“We know that extended exposure to UV has been linked to many eye problems, conditions and in some cases thought to quicken the onset of eye diseases such as Cataract, Age Related Macular Degeneration (AMD), certain cancers on the delicate skin around the eye, Pterygia-a growth of white tissue on the surface of the eye-and even Photokeratitis, a type of sunburn of the cornea”, said JoAnn C. Chang, M.D., a Corneal Specialist at D’Ambrosio Eye Care.
